I understand that there is an inherent risk of injury when choosing to participate in any physical exercise, sport, wellness, and/or recreational activities. My participation is a voluntary activity in all respects and I assume all risks of injury and illness that may result from such participation in any individual activities.

I certify that I am physically fit to participate in fitness and exercise programs or services offered by Momentus Coaching (the “Programs”) and have not been advised to not participate in any of the Programs by a qualified medical professional. I hereby certify that there are no health-related reasons or problems which preclude my participation in any of the Programs.

I understand that the coaching services I will be receiving from my Coach are not offered as a substitute for professional medical care or mental health care, and are not intended to diagnose, treat or cure any mental health or medical conditions. I also understand that my Coach is not acting as a mental health counselor or a medical professional.

I understand and agree that I am fully responsible for my well-being during my coaching sessions and subsequently, including my choices and decisions. I understand that coaching is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care or substance abuse treatment, and I will not use it in place of any form of therapy.

I understand that all comments and ideas offered by my Coach are solely for the purpose of aiding me in achieving my defined goals. I have the ability to give my informed consent, and hereby give such consent to my coach to assist me in achieving such goals.

I understand that the use of technology is not always secure and I accept the risks of confidentiality in the use of email, text, phone, video conferencing, and other technology.

In consideration of my participation in any of the Programs, I do hereby waive, release and discharge Momentus Coaching and its directors, officers, owners, employees, representatives and agents, as well as Wickfield Properties and the Peaceful Dragon School (collectively, Releasees”) from any and all liabilities, claims, demands or damages arising from my participation in any of the Programs, including, without limitation, those caused while practicing on my own or those caused in whole or in part by the negligence of the Releasees. I will indemnify, save and hold harmless each of the Releasees from any loss, liability, claim, damages or cost, including reasonable attorneys’ fees, arising from my participation in any of the Programs.
